Insurance and Fees

Dr. Silvertooth’s practice is fee-for-service and payment is due at the time of visit. Please call the office for fees.

Dr. Silvertooth is not on any medical insurance plans but can provide an itemized receipt at the time of office visits, which individuals may submit to their insurance plans for out of network reimbursement

It is recommended that patients contact their insurance provider prior to their first appointment to determine how much they will be reimbursed for out-of-network provider appointments.

Dr. Silvertooth is OPTED-OUT of MEDICARE and individuals with Medicare or those who are Medicare-eligible (65 years old and older) are required to sign a private contract of service with Dr. Silvertooth. This contract indicates acknowledgement that the individual will not and cannot receive any reimbursement from Medicare for payment provided to Dr. Silvertooth by the individual.
